Co/Extracurricular Participation Forms
Students in grades 9-12 who wish to participate in a sport or club are expected to view the code video and submit the required form(s) before participation may begin.
Juniors - must turn in the code form by October 1 to be eligible to be voted prom king or queen
Seniors - must turn in the code form by October 1 to be eligible to attend the senior trip
Code Form - The code video should be watched by student and a parent/guardian, then fill out the on-line code form and submit it.
Athletes also need one of these forms:
Medical Eligibility Form or Alternate Year Athletic Permit Form - call 608-764-5431 ext. 8100 to find out which form you need
