Co/Extracurricular Participation Forms
Students in grades 6-8 who wish to participate in a sport or club are expected to view the code video and submit the required form(s) before participation may begin.
Code Form - there is no in-person meeting this year and no paper code form. The code video below should be watched by student and a parent/guardian, then fill out the on-line code form and submit it.
Athletes also need one of these forms:
