How to Register

Welcome to the Deerfield Community School District!

To register NEW students in the Deerfield Community School District you must provide the following:

• Proof of Residency (utility bill, signed lease agreement, or signed accepted offer to purchase)
• Birth Certificate, Baptismal Certificate or Passport (for birth date and legal name verification only, the school district does not keep a copy)
• Deerfield Community School District Enrollment Form.  This form is available on this web site under registration forms or at the District Office in the High School.
• Deerfield Community School District Authorization to Obtain and Disclose Information Form.  This form will authorize prior school district to release student's records to the Deerfield Community School District.  This form is available on this web site under registration forms or at the District Office in the High School.
• Home Language Survey.  This form will assist school personnel determine if language assistance will be needed and in completing State Reports.  This form is available on this web site under registration forms or at the District Office in the High School.

New or prospective families are welcome to contact individual schools for building tours and school information.  However, all enrollment and registration processing is centralized through the district office.

All families buying or building a home in the Deerfield township area should contact the Deerfield Community School District Registrar, Molly Poirier at (608) 764-5431, to verify if enrollment is possible with the assigned attendance areas.
